All About Me

Build a one-page profile of your child that you can hand to anybody new — a therapist, a teacher, a dentist, a swimming instructor, a babysitter. Free to use, whether or not your child comes to us.

Nothing you type here is saved or sent anywhere

This tool runs entirely on your own device. We never see what you write, nothing is stored, and there is no account to make. When you close the page it is gone, so print or save the sheet before you leave.

Fill this in

Skip anything that does not apply. Blank sections simply disappear from the sheet. If your child can help fill it in, even better — it works best in their own words.

Who they are
What is good about them

This comes first on purpose. Anybody meeting your child should know who they are before they know what they struggle with.

Where families use this

Parents tell us they end up explaining the same things to every new person they meet. This is so you only have to write it once.

A new school yearHand it to the teacher and the educational assistant in September, before anything goes wrong.
Medical appointmentsDentists, hospitals and blood tests go very differently when staff know in advance what helps.
New therapistsBring it to your first session with us, or with anyone else. It saves a whole appointment of questions.
Babysitters and familyGrandparents and sitters often want to help and are not sure how. This tells them.
Camps and clubsSwimming, soccer, march break camp. Most coaches have never been given this information before.
Anyone new at allA haircut, a birthday party, a first sleepover. Keep a printed copy in your bag.

You do not have to be one of our families to use this. If it helps a child in Ontario have an easier day, that is enough for us.
